How It Works (Formula & Method)
The total-to-HDL ratio divides total cholesterol by HDL ("good") cholesterol; lower is better, with an optimal value under about 3.5:1 and increasing risk above 5:1. The LDL-to-HDL and triglyceride-to-HDL ratios refine that picture, with a TG:HDL ratio under 2 considered favourable.
Non-HDL cholesterol is simply total cholesterol minus HDL. It captures all the cholesterol carried by potentially harmful particles (LDL, VLDL, and others) and is increasingly used as a risk target because it does not require fasting.

Ratios add context that single numbers miss — a high total cholesterol driven by high HDL is very different from the same total driven by high LDL. Values are in US units (mg/dL); to convert from mmol/L, multiply cholesterol by 38.67 and triglycerides by 88.57. Always interpret results with your clinician.
